Insurance & Reimbursement Information

At My Home Circumcision, we provide a safe, gentle, and meaningful in-home circumcision experience performed by a trained and experienced Certified Mohel. While our service is not billed as a medical procedure, many families are still able to submit a claim to their insurance provider for potential reimbursement.

This page outlines what you can expect and exactly what information to provide to your insurance company.

Are We In-Network or Out-of-Network Providers?

We are not considered a medical provider for insurance purposes.

  • We do not bill insurance.
  • We do not have a medical NPI number.
  • We do not have a state medical license because the service is performed as a religious ceremony, not as a medical procedure.

Families pay for the service directly, then may choose to submit for reimbursement depending on their plan.

Who Performs the Circumcision?

All procedures are performed by:

Rabbi Gadi Levy - Certified Mohel

Certified by the National Organization of American Mohels (NOAM)

Trained in newborn circumcision techniques and aftercare

Because the circumcision is performed under religious practice, it is not billed as medical care, even though extensive training and safety standards are followed.

Information You Can Provide to Your Insurance Company

Insurance companies often ask for a license number, NPI number, or provider information.

Here is what you may use:

  • Provider Name: Rabbi Gadi Levy (Gad Levi)
  • Title: Certified Mohel
  • Certification: National Organization of American Mohels (NOAM)
  • Service Type: Home-based religious circumcision
  • NPI Number: Not applicable
  • Medical License: Not applicable

Most insurance companies will accept this explanation and process your claim according to your plan’s benefits.

How to Submit for Reimbursement

After your service, you will receive a detailed receipt that you can submit with a claim.

Families typically include:

  1. Your invoice/receipt from My Home Circumcision
  2. Our Insurance Information Sheet DOWNLOAD HERE
  3. A note such as:
    “This was a home-based newborn circumcision performed by a Certified Mohel as part of a religious service. The provider does not hold a medical NPI. Please review for any eligible reimbursement according to my plan benefits.”

Your insurance company will then evaluate your claim based on your out-of-network or exception benefits.

If Insurance Requests a Procedure Code

Some families choose to include a CPT code when submitting their claim.

The code most commonly used is:

Z41.2 - Encounter for routine and ritual male circumcision

Provided as a reference only. My Home Circumcision does not bill this code.
